In-Home Care Services

Our In-Home Care Services are designed to help seniors and individuals with disabilities live safely and comfortably in their own homes. We provide compassionate, personalized support with daily activities—promoting independence, dignity, and quality of life.

Services include assistance with personal hygiene, dressing, nutrition, and light housekeeping, along with other essential daily living needs.

We offer a range of programs to meet individual needs, including Medicaid Personal Care, Medicaid Aged and Disabled Waiver, Private Pay, Lighthouse, and FAIR.

Medicaid Personal Care

Medicaid Personal Care services provide physician-ordered support to help individuals remain in their homes rather than moving to a facility. Our trained caregivers assist with essential daily tasks, ensuring comfort and well-being.

Services include:

  • Personal hygiene
  • Dressing and feeding
  • Nutritional support
  • Light housekeeping and environmental support

Medicaid Aged and Disabled Waiver (ADW)

The ADW Program offers in-home and community-based services for eligible individuals age 18 and older. This program is a long-term care alternative to nursing home placement, helping individuals maintain independence while receiving the care they need.

Our person-centered approach focuses on choice, dignity, and community connection.

Services include:

  • Case Management: Personalized care planning and connection to community resources
  • Homemaker Services: Assistance with daily living activities and household tasks
  • Transportation: Access to medical appointments and community services
  • RN Assessments: Ongoing evaluations to ensure care plans meet individual needs.

Private Pay Services

Our Private Pay Services offer flexible, personalized care options for individuals and families who need support at home. Services may include homemaker assistance, case management, and RN assessments.

Costs vary based on the level and type of care needed. As a nonprofit organization, we are committed to offering competitive rates and working closely with families to ensure each individual receives the support they deserve.

Lighthouse Program

The Lighthouse Program helps seniors remain independent and continue living safely at home. Established in 2007, this program provides essential support services that enhance quality of life and reduce the need for institutional care.

Eligible individuals may receive up to 60 hours of service per month.

Eligibility requirements:

  • Age 60 or older
  • Medical eligibility determined by an RN assessment
  • Financial eligibility based on a sliding fee scale

Services include:

  • Personal Care: Bathing, grooming, dressing, toileting
  • Mobility Support: Assistance with walking and transfers
  • Nutrition: Meal preparation, eating, grocery and pharmacy trips
  • Home Support: Light housekeeping, laundry, and bed-making

FAIR (Family Alzheimer’s In-Home Respite)

Caring for a loved one with Alzheimer’s or dementia can be overwhelming. The FAIR Program provides caregivers with much-needed time to rest and recharge.

Eligible families can receive up to 16 hours of respite care per week, allowing caregivers to attend appointments, run errands, or simply take a break.

During this time, a trained caregiver provides safe, compassionate support in the home, engaging the individual with meaningful activities and social interaction tailored to their needs.

Veteran’s Administration Assistance

Veterans may qualify for in-home care services through the VA. Eligibility is determined by the VA clinic, and services begin with a referral to PRIDE.
